Who are we?
Prime Care Community Services (“Prime Care”) is a leading, award-winning provider of home-care services to adults and older people across Sussex.
Care Co-ops Community Development Service (“Care Co-ops”) is a community interest company renowned for its innovative approach to supporting and empowering people in a range of community and work-focussed settings.
This exciting new partnership between Care Co-ops and Prime Care draws on the combined strength and experiences of both organisations within the adult social care field.
What do we offer?
We deliver the flexibility of a Personal Assistant (“PA”) to support you in making a life for yourself which reflects your own choices. Our supportive and skilled team will work with you, so you can access opportunities and gain control your daily life.
Our new Community PA Service manages the employment, training, supervision, and verification of your individual Community PA or team of PAs. We make sure everything is in-place so that you can enjoy what you want to do. The service will select the best PA or team for your support needs, so that you can get on with what you want to do.
We call this our fully-managed service which means that you can get on with your goals and wishes and we look after the employment side so you don’t have to worry.
What will your Community PA do?
Your Community PA worker or team, are skilled and motivated to assist you in participating in the widest possible range of activities. Things like swimming; going out to eat and drink at cafes and bars; walks at your favourite locations; participating in adult and further education; travelling and sight- seeing, visiting friends and relatives – and many other daily life activities you want to do.
Your PA can assist you in one-off activities, a series of events, or more regular assistance in your work or social activity. We can do this during the day, at weekends or in the evenings. The main aim is to give you choice, access and control. We can also assist you in your own home, with your daily living and care needs: which is provided by Prime Care’s domestic care team.
Who do we work with?
We can work with all adults over 18. This includes older people, those with physical and sensory disabilities, people with mental health and support needs, individuals with a learning disability, and anyone who needs rehabilitation or day-to-day support.
In fact, we will work with any adult who has a support need whether they are privately funded or have a direct payment or individual budget, or are funded by other means.
We work across East Sussex, Brighton and Hove, and West Sussex.
We also welcome referrals of clients and service users? by health and social care professionals, adult social care departments and primary care trusts.
